Roles & Responsibilities

• Coordinate comprehensive care plans and manage discharge planning for acute care patients
• Perform utilization review to ensure appropriate, timely care and safe transitions
• Arrange DME, SNF placements and ambulance transport independently and efficiently
• Lead and participate in family meetings to align care goals and discharge plans
• Collaborate with interdisciplinary team members (PT, Social Services, etc.) to support patient needs
• Document care coordination and case activity accurately using Epic
• Assess patient needs including social determinants, behavioral health history, and community resources
• Facilitate prior authorizations and work with HMOs/IPAs to secure necessary approvals

Qualifications

• Active California state license required (pending licenses accepted but must be active by start date)
• Minimum 2 years acute care case management/discharge planning experience in a hospital setting
• Demonstrated hospital experience — behavioral health-only background does not meet this requirement
• Travel/contract experience required — seasoned traveler only (first-time travelers not accepted)
• Proficiency with Epic (charting system required)
• Ability to meet patient ratios (approx. 1:25 on days; higher on weekends) and work every other weekend
• Strong organizational skills to coordinate care, prior authorizations, and placements independently
• Willingness to complete a 40-hour orientation and adhere to RTO restriction of no more than 7 days

Knowledge & Skills

• Care coordination and discharge planning expertise
• Utilization review and prior authorization processes
• DME, SNF and ambulance transport coordination
• Familiarity with CMS, DRG, and The Joint Commission/core measures
• Experience with HMOs/IPAs and benefit eligibility processes
• Skill in conducting family meetings and contributing to treatment plan development
• Knowledge of community resources (foster/adoption, substance-use treatment, homelessness, mental health, protective services, POLST/advance directives)
