Roles & Responsibilities

• Assess patients and observe treatment effects to guide ongoing care
• Perform thorough patient evaluations and communicate recommended changes to the care team
• Develop and implement individualized treatment plans to restore function and independence
• Select and deliver appropriate therapeutic procedures for neuromuscular, skeletal, integumentary, pulmonary and cardiovascular conditions
• Collaborate with physicians, nurses and interdisciplinary staff to ensure coordinated patient care
• Document patient progress clearly and provide education and support to patients and families
• Contribute to a positive, patient-centered environment that promotes rehabilitation and recovery

Qualifications

• Current Colorado Physical Therapy license (required)
• Basic Life Support (American Heart Association) certification (required)
• Willingness to relocate to a rural community (required)
• Compassionate, patient-focused approach and strong communication skills
• Experience with rehabilitation and therapeutic procedures preferred

Knowledge & Skills

• Patient assessment and treatment planning
• Evaluating treatment effect and adjusting care as needed
• Therapeutic procedures for rehabilitation and functional restoration
• Experience with neuromuscular, skeletal, integumentary, pulmonary and cardiovascular disorders
• Interdisciplinary collaboration and clinical documentation
