Roles & Responsibilities

• Provide comprehensive home health nursing care including admission assessments
• Perform thorough patient evaluations to determine clinical needs
• Develop, document and implement individualized care plans
• Coordinate care with an interdisciplinary team of homecare professionals and paraprofessionals
• Educate patients and families on care plans, medications, and self-care strategies
• Document clinical findings and progress accurately and promptly
• Support hospice patients with compassionate, dignified care when appropriate
• Advocate for patient safety and high-quality coordinated care across Caledonia and southern Essex counties

Qualifications

• Current Vermont RN license (required)
• Registered Nurse credential with home health or clinical nursing experience
• Minimum of 1 year of prior clinical experience (required)
• Experience with admission, patient evaluation, and care plan development
• Comfort working with patients of all ages in home settings and collaborating with a care team
• Compassionate, team-oriented approach to patient care

• Home health nursing best practices and community-based care
• Patient assessment and clinical evaluation
• Care plan development and individualized care delivery
• Admission procedures and initial patient assessments
• Skilled documentation and clinical communication
• Experience supporting hospice care and end-of-life needs
