Roles & Responsibilities

• Perform in-home assessments to identify functional impairments and therapy needs
• Develop and implement individualized plans of care that promote safety, mobility, and independence
• Deliver hands-on ADL interventions and home safety solutions tailored to each patient
• Educate patients and caregivers on adaptive equipment, strategies, and fall prevention
• Collaborate with nurses, physical therapists, aides, and physicians to ensure coordinated care
• Document clinical findings, progress, and plan updates in the EMR or mobile charting system
• Support safe transitions from hospital to home and advocate for long-term functional wellness

Qualifications

• Licensed Occupational Therapist (OTR/L) in the state of New Mexico
• Minimum of 1 year of clinical experience (home health experience preferred)
• Experience with EMR systems and mobile charting tools
• Strong communication, time management, and problem-solving skills
• Comfort working independently in patients' homes and managing a home health caseload
• Passion for enabling independence, dignity, and patient-centered outcomes

Knowledge & Skills

• Skilled in ADL interventions and adaptive techniques
• Proficient in home safety assessments and fall prevention strategies
• Experienced in patient assessment and plan of care development
• Strong documentation practices and use of EMR/mobile charting
• Care coordination and interdisciplinary collaboration
• Excellent verbal communication and patient/caregiver education skills
• Time management and independent clinical decision-making
