Roles & Responsibilities

Provide compassionate, hands-on nursing care to clinic patients following established standards and scope of LPN practice.
Perform triage of patient phone calls and determine appropriate next steps for timely care.
Take and document vital signs accurately to support clinical assessments and provider decision-making.
Manage medication refills and complete necessary prior authorizations to keep patients on track with therapy.
Serve as the primary liaison between patients, providers, and interdisciplinary team members to coordinate care.
Give direction to CNAs and schedulers and collaborate closely with providers, coordinators, clinical managers/supervisors, and RNs.
Maintain clear, timely documentation and support quality and safety initiatives in the clinic.
Stay current with continuing education and licensure requirements to ensure best-practice patient care.

Qualifications

Current State LPN license required
BLS (Basic Life Support) certification required
Graduate of a Nursing Program
Demonstrated experience with vital signs, triage, medication refills, and prior authorizations
Ability to provide supervision and direction to CNAs and schedulers
Strong communication skills and ability to work collaboratively with an interdisciplinary team
Willingness to occasionally work weekends

Education

Graduate of a Nursing Program
LPN diploma or equivalent

Knowledge & Skills

Patient care and bedside fundamentals
Triage and telephone triage protocols
Taking and interpreting vital signs
Medication management and prescription refill workflows
Completing prior authorizations and paperwork
Basic Life Support (BLS)
Delegation and supervision of CNAs and schedulers
Interdisciplinary team collaboration and patient communication
