Candidate Pay Rate: $67 per hour

Roles & Responsibilities
- Evaluate patients with thoughtful assessments to create individualized treatment plans
- Implement therapeutic exercises, gait training, and mobility programs to improve function
- Provide hands-on manual therapy and use appropriate modalities when indicated
- Collaborate with nursing, OT, SLP, and the interdisciplinary team to coordinate care
- Document progress clearly and timely in the facility EMR to support continuity of care
- Educate patients and families on home exercise programs, safety, and community resources
- Participate in discharge planning to ensure safe transitions back to home or next level of care
- Support unit goals by mentoring staff and contributing to a positive work environment
Qualifications
- Active Physical Therapist license (state license required; MA license preferred or eligible)
- Graduate degree in Physical Therapy (DPT preferred) or equivalent
- BLS/CPR certification required, other certifications a plus
- Experience in skilled nursing, inpatient rehab, or geriatric care preferred
- Strong documentation skills and comfort with electronic medical records
- Excellent communication and teamwork skills; able to work across all seven days
Education
- Doctor of Physical Therapy (DPT) preferred
- Master's or Bachelor's in Physical Therapy accepted with relevant experience
Knowledge & Skills
- Thorough knowledge of therapeutic exercise, functional mobility, and gait training
- Skilled in patient assessment, goal setting, and outcome measurement
- Familiarity with assistive devices, transfers, and fall prevention strategies
- Proficient with documentation standards and skilled nursing documentation
- Ability to triage safety risks and modify plans for complex medical needs
- Strong interpersonal skills for patient and family education
