Roles & Responsibilities

• Provide compassionate, patient-centered nursing care as an LPN in long-term care and rehab settings
• Administer medications and document accurately and timely
• Monitor and report changes in patient condition to RN leadership and providers
• Assist with ADLs and support resident mobility and dignity
• Collaborate with interdisciplinary team to implement individualized care plans
• Participate in shift handoffs and maintain clear, concise charting

Qualifications

• Active LPN license (state of practice required or eligible for licensure)
• Current Basic Life Support (BLS) preferred or willingness to obtain
• Previous experience in long-term care or similar clinical setting preferred
• Ability to pass required background checks and health screenings
• Strong communication skills and a team-oriented mindset

Education

• Graduate of an accredited practical nursing (LPN/LVN) program
• Diploma or certificate in Practical Nursing

Knowledge & Skills

• Medication administration and basic clinical assessments
• Charting and electronic health record familiarity
• Infection control practices and safe patient handling
• Time management, prioritization, and reliable follow-through
• Positive bedside manner and family communication skills
