Roles & Responsibilities
- Provide compassionate direct patient care and support patients' daily needs
- Administer medications and treatments safely and according to policy
- Perform timely assessments and monitor patient condition, reporting changes promptly
- Document care accurately in the chart and maintain clear communication with the team
- Collaborate with nurses, providers and support staff to deliver coordinated care
- Assist with patient education and discharge planning to promote positive outcomes
Qualifications
- Current Licensed Practical Nurse (LPN) license
- Active basic life support (BLS/CPR) certification or willingness to obtain
- Comfortable working in a fast-paced clinical setting and adapting to per diem shifts
- Strong communication skills and a patient-first attitude
- Previous LPN experience preferred but new grads with clinical strength are encouraged to apply
Education
- LPN diploma or equivalent vocational nursing program completion
Knowledge & Skills
- Clinical assessment and monitoring of vital signs
- Medication administration and safe documentation practices
- Wound care basics and assistance with ADLs
- Electronic medical record (EMR) familiarity and accurate charting
- Team collaboration, time management and patient-centered communication
