Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients with dignity and respect
- Administer medications and treatments accurately and document per policy
- Monitor and record vital signs, changes in condition, and communicate concerns promptly
- Collaborate with the interdisciplinary team to develop and carry out patient care plans
- Assist with wound care, ADLs, and other clinical procedures within LPN scope
- Maintain clear, timely charting in the facility’s medical record system
Qualifications
- Current, active LPN license in New Hampshire or eligible for licensure
- Current BLS/CPR certification (or obtain upon hire)
- Comfortable working 12-plus hour shifts and adapting to per-diem scheduling
- Strong communication skills and a team-first attitude
- Prior clinical experience in long-term care, rehab, or acute care preferred
Education
- Graduate of an accredited LPN/LVN program or equivalent vocational nursing program
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and dressing changes
- Accurate vital sign assessment and clinical observation
- Familiarity with electronic medical records and charting
- Patient-centered communication and family liaison skills
- Time management and prioritization in fast-paced clinical settings
