Roles & Responsibilities

• Provide compassionate day-to-day nursing care to patients, ensuring safety and comfort
• Administer medications and treatments accurately according to plan of care
• Monitor and document patient status, vital signs, and clinical changes
• Collaborate with nurses, physicians, and care team members to deliver coordinated care
• Perform procedure support and wound care with attention to infection control best practices
• Educate patients and families about care plans, medications, and discharge instructions
• Maintain clear, timely documentation in the patient chart and communicate handoffs effectively

Qualifications

• Active LPN license in good standing (required)
• Current CPR/BLS certification (or willingness to obtain promptly)
• Clinical experience as an LPN in acute or post-acute settings preferred
• Up-to-date immunizations and ability to complete facility onboarding requirements
• Strong communication, teamwork, and patient-centered care skills
• Comfortable working flexible per diem shifts and adapting to variable assignments

Education

• LPN diploma or certificate from an accredited program
• Active state LPN licensure

Knowledge & Skills

• Solid understanding of clinical assessments and basic nursing interventions
• Medication administration and documentation accuracy
• Ability to prioritize tasks in a fast-paced environment
• Skillful patient and family education with empathy and clarity
• Familiarity with electronic health record documentation
• Team-oriented approach with strong clinical judgment
