Contract Type: Travel Contract

Shift Type: Monday-Friday, 7:00 AM - 5:30 PM (Day shift); schedule options include 5x8 or 4x10 within site hours (flexible schedule, no weekends)

Hours per Week: 36

Candidate Pay Rate: $74 per hour

Roles & Responsibilities

• Provide high-quality outpatient physical therapy care across the lifespan, from pediatrics to geriatrics
• Assess, plan, and implement individualized treatment plans for orthopedic, neurological, vestibular, pelvic health, stroke, and work comp patients
• Collaborate closely with PTAs, occupational therapy, and speech therapy for co-treatment and coordinated care
• Document patient progress clearly and timely to support continuity of care
• Educate patients and families on home programs, injury prevention, and functional goals
• Participate in team meetings and contribute to a positive, patient-first clinic culture

Qualifications

• Active Physical Therapist license in Illinois (or eligible to obtain)
• Degree in Physical Therapy (Master's or Doctorate preferred)
• Experience or interest in outpatient settings including orthopedics, neuro/stroke rehab, vestibular and pelvic health
• Comfort collaborating with PTAs and other therapy disciplines (OT, Speech)
• Strong communication skills and ability to document effectively
• BLS/CPR certification preferred or willingness to obtain
• Ability to work the flexible day schedules described (5x8 or 4x10) and adapt to varied patient populations

Education

• Doctor of Physical Therapy (DPT) preferred; Master of Physical Therapy or equivalent considered
• Relevant state licensure required

Knowledge & Skills

• Skilled in orthopedic assessment and rehabilitation techniques
• Experience with neurological/Stroke rehab principles
• Vestibular rehabilitation assessment and treatment knowledge
• Pelvic health evaluation and treatment experience or interest
• Familiarity with pediatrics through geriatrics across outpatient caseloads
• Ability to collaborate effectively with PTAs, OT, and Speech colleagues
• Strong clinical reasoning, patient education skills, and documentation proficiency
