Roles & Responsibilities

• Assess patients and caregivers using the nursing process to create individualized, evidence-based plans of care
• Develop, document and communicate realistic care plans that include patients and caregivers in decision-making
• Serve as the case manager for assigned patients, actively managing caseloads and visit timing to meet productivity standards
• Complete accurate, timely OASIS documentation and stay current on OASIS requirements
• Implement medical plans of care and nursing interventions to ensure safe, high-quality home care
• Perform and maintain high-tech skills such as IV use and maintenance, wound VAC management, and complex wound care
• Coordinate care with interdisciplinary team members, physicians and vendors to support the recommended plan of care
• Supervise and delegate to LPNs within their scope, review documentation, and act on recommendations
• Complete required agency, state and federal forms (HHABN, Medicare Notice of Non-Coverage) in a timely manner
• Facilitate safe discharges/transfers and communicate essential patient information to ensure continuity of care

Qualifications

• Active Registered Nurse (RN) — ready to practice in Vermont and comfortable working in home health
• OASIS experience REQUIRED — you know the documentation expectations and timelines
• Demonstrated case management experience and ability to manage caseloads effectively
• Comfortable with IV therapy, wound VACs and complex wound care
• Experience with McKesson processes or similar clinical documentation systems
• Ability to delegate and collaborate with LPNs and other team members
• Strong documentation, patient education and interdisciplinary coordination skills

Education

• Nursing degree (ADN or BSN) — active RN license required

Knowledge & Skills

• Thorough knowledge of home health nursing assessment and care planning
• Strong documentation practices and timeliness with OASIS and agency policies
• Infection control best practices and quality improvement reporting
• Skillful patient and caregiver education with an emphasis on safety and self-management
• Ability to coordinate services across disciplines (PT, social work, vendors) and manage transitions of care
• Familiarity with HHABN, Medicare Notice of Non-Coverage and other required forms
