Roles & Responsibilities
- Perform thorough assessments and obtain vital signs to track patient status
- Reconcile medications and review patient histories to promote safe care
- Collect, label, and process specimens including venipuncture and throat/urine samples
- Set up and assist with procedures, maintain aseptic technique, and perform dressing changes
- Document accurately and efficiently in EPIC to ensure continuity of care
- Provide telephone triage, handle pharmacy refill requests, and complete follow-up calls
- Collaborate with interdisciplinary team members to support comprehensive, community-based care
- Deliver compassionate, patient-centered care tailored to the geriatric population
Qualifications
- Active RN licensure in Washington or compact RN; pending Washington license accepted
- Minimum 2 years of RN experience (experience with geriatric population highly preferred)
- Experience with EPIC charting is required
- Current BLS certification (preferred if not required)
- Skilled in venipuncture/phlebotomy, specimen collection and labeling
- Strong communication skills for patient education, triage, and interdisciplinary coordination
Education
- Registered Nursing credential (RN) required; specific degree (ASN/BSN) not specified
Knowledge & Skills
- Clinical assessment and obtaining vital signs
- Medication reconciliation and interpretation of lab results
- Specimen collection: venipuncture, capillary sampling, throat swabs, urine (UA/Clean Catch)
- Aseptic/sterile procedure setup and dressing changes
- Telephone triage, pharmacy refill coordination, and follow-up phone calls
- Familiarity working in outpatient, family care, internal medicine, urgent care, and medical-surgical settings
- Proficient charting in EPIC and strong documentation habits
- Comfort working with older adults and applying geriatric best practices
