Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and treatments safely and document care accurately
• Monitor patient status, recognize changes, and communicate concerns to the care team
• Support activities of daily living and promote resident dignity and independence
• Collaborate with RNs, physicians, and interdisciplinary team members to deliver coordinated care
• Maintain accurate and timely clinical documentation in the facility charting system
• Participate in shift handoffs and contribute to a positive, resident-focused environment

Qualifications

• Active LPN license in good standing (state authorization to practice required)
• Current Basic Life Support (BLS) certification or willingness to obtain
• Comfortable working evening and overnight hours
• Strong clinical judgment, communication, and teamwork skills
• Ability to pass facility-required background and health screenings

Education

• Completion of an accredited practical nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and infection control principles
• Vital signs assessment and recognizing clinical changes
• Charting and electronic health record familiarity
• Patient-centered communication and family engagement
• Time management and prioritization in a fast-paced care setting
