Roles & Responsibilities
- Provide compassionate, direct patient care and monitor patient condition closely
- Administer medications and treatments safely and accurately according to plan
- Collaborate with the interdisciplinary team to implement individualized care plans
- Perform routine assessments and document clinical findings clearly and timely
- Assist with patient admissions, transfers and discharges to ensure smooth transitions
- Support patient and family education with clear, empathetic communication
- Maintain a safe, clean environment and adhere to infection control practices
Qualifications
- Current Licensed Practical Nurse (LPN) credential in the state of practice
- Valid basic life support (BLS) certification or willingness to obtain
- Recent clinical experience in a similar long term care or skilled nursing setting preferred
- Ability to pass facility onboarding requirements including background checks and immunizations
- Strong communication skills and a team-first attitude
Education
- Completed accredited LPN/LVN program
- High school diploma or GED
Knowledge & Skills
- Proficient in medication administration and basic wound care
- Skilled in patient assessment, vital signs and documentation
- Comfortable using electronic medical records and following care protocols
- Ability to prioritize tasks in a fast paced environment
- Compassionate bedside manner and strong interpersonal skills
