Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents in a long-term care and rehab setting
• Administer medications and treatments safely and accurately following facility protocols
• Assess patients, monitor changes in condition, and document findings thoroughly
• Collaborate with RNs, therapists, and interdisciplinary team members to develop and update care plans
• Support activities of daily living and encourage patient independence and dignity
• Communicate clearly and respectfully with residents and families to keep them informed and involved
• Maintain infection control standards and ensure a safe environment for patients and staff

Qualifications

• Current Licensed Practical Nurse (LPN) license — must be active and in good standing (New Mexico license preferred)
• BLS (Basic Life Support) certification or willingness to obtain prior to start
• Experience in long-term care, skilled nursing, or rehab settings is highly valued
• Strong clinical judgment, attention to detail, and a patient-centered approach
• Excellent communication and teamwork skills

Education

• Graduate of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and accurate charting
• Wound care basics and pressure injury prevention
• Familiarity with electronic health record (EHR) documentation
• Vital signs assessment and basic patient assessment
• Infection control practices and safe patient handling
• Good time management and the ability to prioritize in a busy environment
