Roles & Responsibilities

• Provide safe, patient-centered hemodialysis care in an outpatient/chronic dialysis setting
• Set up, monitor and troubleshoot Fresenius 20008T dialysis equipment and alarms
• Manage vascular access care, including AV fistula and dialysis catheter handling
• Administer blood and blood products and manage anticoagulation per protocol
• Monitor and document vital signs, venous and arterial pressures, ultrafiltration and patient response during treatments
• Complete thorough, timely charting in Epic and communicate care plans with the interdisciplinary team
• Deliver post-dialysis treatments including blood return, disconnect procedures, access care and sterilization
• Coach and educate patients and families about dialysis care and self-management

Qualifications

• Active RN license in California (pending licenses not accepted)
• Minimum 1 year of chronic/outpatient dialysis experience required (hard stop)
• BLS required; CDN and CNN certifications preferred
• Experience with Fresenius 20008T equipment and machine alarm troubleshooting
• Proficiency with Epic charting preferred
• Comfort working 12+ hour day shifts and providing weekend coverage every other weekend
• Charge nurse experience preferred

Knowledge & Skills

• Dialysis treatment and chronic outpatient dialysis workflows
• Machine alarm troubleshooting: air/foam, arterial/venous pressure, blood leak, conductivity, temperature and ultrafiltration alarms
• Blood flow rates (Qb) and dialysate flow rate (Qd) management
• Anticoagulation management and administration of blood and blood products
• Vascular access assessment and malfunction management
• Post-dialysis care and sterilization procedures
• Infection control and blood glucose monitoring
• Familiarity with Epic electronic charting
