Roles & Responsibilities
- Provide compassionate, skilled nursing care to patients in home care and hospice settings
- Perform assessments, administer medications, and document patient status accurately
- Collaborate with interdisciplinary teams to support holistic patient and family needs
- Educate patients and families about care plans, symptom management, and safety
- Complete required skills checklists and maintain up-to-date certifications
- Adhere to infection control, safety protocols, and all Trinity verification requirements
- Maintain clear, timely documentation and communicate changes in patient condition
Qualifications
- Active Licensed Practical Nurse (LPN) license (must be verifiable)
- BLS (Basic Life Support) certification required
- Ability to pass Trinity Skills Checklist and license/certificate verification
- Up-to-date health requirements: Fit Test; 2-step PPD or IGRA (Quantiferon/Tspot)
- Required immunizations/screens: MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza vaccine
- Clean Trinity 10 panel drug screen and completed physical/health statement
- Experience in home care or hospice settings is a strong plus
- Reliable transportation and ability to travel locally for patient visits
Education
- Completion of an accredited practical nursing (LPN/Vocational Nursing) program and active LPN licensure
Knowledge & Skills
- Proven clinical skills in medication administration, wound care, and patient assessment
- Comfort working independently in patient homes with strong critical thinking
- Compassionate communication with patients, families, and interdisciplinary teams
- Time management and documentation skills to keep accurate, timely records
- Familiarity with hospice principles and home care best practices
- Ability to follow infection control practices and complete required health screenings
