Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
- Perform assessments and monitor residents’ vital signs and clinical status
- Administer medications and treatments safely and accurately according to policy
- Document care, observations and changes in condition clearly and timely
- Collaborate with nursing team and interdisciplinary staff to coordinate resident care
- Support resident activities of daily living and promote a comfortable environment
- Communicate with residents and families with empathy and professionalism
- Participate in required competency testing and maintain compliance with facility protocols
Qualifications
- Active Licensed Practical Nurse (LPN) license — license verification required
- BLS (Basic Life Support) certification required
- Must meet Trinity-specific competency testing requirements
- Current immunizations and health screenings as listed by the facility (PPD/IGRA, MMR, Hep B, Varicella, Tdap, Influenza, COVID-19 as required)
- Fit test and respirator evaluation completion
- Successful completion of Trinity 10-panel drug screen and health statement
Education
- Graduate of an accredited Practical Nursing (LPN) program
Knowledge & Skills
- Solid knowledge of medication administration and basic nursing procedures
- Strong clinical assessment and documentation skills
- Infection control practices and safe use of PPE
- Effective communication and team collaboration
- Ability to follow facility policies, protocols and competency standards
- Comfort working in a per diem/pool environment with variable schedules
