Shift Type: Per Diem / Pool, variable shifts (day, night, and weekends as needed)

Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents consistent with LPN scope of practice
- Administer medications and treatments accurately and document thoroughly
- Monitor resident conditions and communicate changes to the care team promptly
- Support activities of daily living and encourage resident independence
- Participate in competency testing and maintain required certifications
- Maintain a safe, clean environment and follow infection control protocols
- Collaborate with nursing staff and leadership to deliver person-centered care
Qualifications
- Current Licensed Practical Nurse (LPN) license — required
- BLS (Basic Life Support) certification — required
- Completion of Trinity Specialty Specific Competency Testing (per checklist)
- Successful license/certificate verification on file
- Ability to complete required health screenings: fit test, 2-step PPD or IGRA, influenza vaccine, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine
- Negative Trinity 10 panel drug screen and completed physical/health statement
- Trinity respirator evaluation and Trinity color vision testing as applicable
- Strong communication skills and a team-first attitude
Education
- Graduate of an accredited LPN / Vocational Nursing program
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Knowledgeable about infection prevention and resident safety practices
- Ability to accurately document care in resident charts and electronic records
- Comfortable working in long-term care / assisted living environments
- Good clinical judgment and ability to escalate concerns appropriately
- Teamwork, time management, and interpersonal skills
