Roles & Responsibilities

Conduct comprehensive psychosocial assessments to evaluate patients' mental health needs
Develop and implement individualized treatment plans in collaboration with interdisciplinary teams
Provide crisis intervention and 5150 evaluations for involuntary psychiatric holds
Coordinate care transitions, including discharge planning and referrals to community resources
Advocate for patients' rights and access to mental health services
Document clinical interventions and maintain compliance with regulatory standards

Qualifications

Current Licensed Clinical Social Worker (LCSW) in California
Current BLS certification
5150 certification (must be obtained prior to start if not already held)
Prior experience in a Dignity Health or CommonSpirit facility (required)
Minimum of 2 supervisory references, with at least one within the past 12 months

Education

Master's degree in Social Work (MSW) from an accredited program

Knowledge & Skills

In-depth knowledge of California 5150 laws and involuntary commitment procedures
Strong crisis assessment and intervention skills
Ability to collaborate effectively with healthcare teams and community partners
Excellent documentation and communication skills
Familiarity with care coordination and discharge planning processes
