Roles & Responsibilities

• Assess and evaluate patients' physical therapy needs in the comfort of their homes
• Develop and implement personalized treatment plans to restore mobility and function
• Educate patients and families on therapeutic exercises and equipment use
• Collaborate with physicians and healthcare team to optimize patient outcomes
• Document patient progress and adjust care plans as needed
• Supervise home health aides and ensure quality care delivery
• Participate in case conferences and staff competency assessments

Qualifications

• Active Physical Therapist license in California (or compact eligible)
• Minimum 2 years of experience in acute care, home health, or rehab setting
• Current BLS certification from the American Heart Association
• Strong communication and organizational skills to thrive in a home health environment
• Ability to work independently and manage a caseload across Yolo County

Knowledge & Skills

• Expertise in physical therapy assessments and treatment modalities
• Excellent verbal and written communication for patient and team interactions
• Organizational skills to manage multiple patients and documentation
• Compassionate approach to patient care in home settings
• Understanding of home health regulations and Medicare guidelines
