Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care, recertifications, and resumption of care visits in home health settings
Develop and implement individualized physical therapy treatment plans
Complete OASIS assessments and documentation accurately
Collaborate with interdisciplinary team for coordinated patient care
Provide patient and family education for optimal outcomes
Maintain productivity of 6.5 points per day

Qualifications

Current North Dakota Physical Therapy License
Basic Life Support (BLS) certification from AHA
Valid Driver's License and Auto Insurance
Minimum 1 year of home health experience preferred
OASIS training and experience strongly desired
First-time travelers welcomed with appropriate experience

Knowledge & Skills

Home health and hospice regulatory knowledge
OASIS assessment and documentation skills
Ability to work independently in the field
Strong communication and time management
Proficient in EHR systems
