Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct comprehensive patient evaluations and develop individualized treatment plans for home health patients
Implement therapeutic exercises and modalities to improve mobility, strength, and function
Educate patients and families on home exercise programs and fall prevention strategies
Collaborate with interdisciplinary care team to ensure coordinated patient care
Document patient progress accurately in HomeCare/HomeBase EMR system
Manage caseload of 5-7 patients per day within a 60-mile radius of Valley City

Qualifications

Current North Dakota Physical Therapy License required
Basic Life Support (BLS) certification required
Valid driver's license and auto insurance in good standing
Minimum 1 year of physical therapy experience, preferably in home health
Proficiency with HomeCare/HomeBase EMR system preferred
Comfortable traveling up to 60 miles for patient visits

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Knowledge of home health regulations and documentation standards
Ability to work independently and manage a flexible daily schedule
Experience with orthopedic, post-surgical, and general rehabilitation cases
