Candidate Pay Rate: $59 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes, helping them regain independence
Develop and implement individualized treatment plans based on patient assessments
Educate patients and their families on exercises and home safety to prevent falls
Collaborate with physicians, nurses, and other healthcare professionals to coordinate care
Document patient progress and maintain accurate records in compliance with regulations

Qualifications

Active California Physical Therapy license required
Current BLS certification from the American Heart Association
Minimum 1 year of home health or relevant physical therapy experience preferred
Strong clinical assessment and critical thinking skills
Excellent communication and patient education abilities

Knowledge & Skills

Comprehensive knowledge of physical therapy principles and practices
Proficiency in electronic medical records and documentation
Ability to work independently and manage time effectively
Empathy and patience when working with diverse patient populations
