Candidate Pay Rate: $85 - $59.5 per hour

Roles & Responsibilities

Evaluate residents and develop individualized physical therapy treatment plans that promote mobility and independence
Provide targeted therapy focused on strength, balance, gait, and functional recovery
Monitor resident progress, adjust plans as needed, and document outcomes clearly
Collaborate closely with physicians, nurses, and the interdisciplinary team to coordinate care
Educate residents and caregivers on home exercise programs, safe mobility strategies, and injury prevention
Champion resident-centered care and contribute to a positive, rehabilitative environment

Qualifications

Active California Physical Therapist License (required)
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program (required)
Previous SNF, rehabilitation, or geriatric therapy experience preferred
Strong clinical assessment and patient care skills
Excellent communication and documentation abilities
Comfortable working as part of a multidisciplinary care team

Education

Knowledge & Skills

Expertise in mobility training and rehabilitation techniques
Proven skills in patient evaluation and individualized treatment planning
Knowledge of strength, balance, and functional recovery approaches
Clear, compassionate communication with residents, families, and team members
Accurate clinical documentation and time management
Collaborative approach to interdisciplinary care
