Roles & Responsibilities

Evaluate residents and develop individualized physical therapy treatment plans that promote independence
Provide hands-on therapy focused on mobility, strength, balance, and functional recovery
Monitor resident progress and adjust treatment plans to achieve meaningful outcomes
Collaborate with physicians, therapists, and the interdisciplinary team to deliver coordinated care
Maintain accurate, timely clinical documentation to support quality care and communication
Educate residents and caregivers on exercises, mobility techniques, and fall prevention strategies

Qualifications

Active California Physical Therapist License (required)
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
Previous SNF, rehabilitation, or geriatric therapy experience preferred
Strong clinical assessment and patient care skills
Ability to work effectively within a multidisciplinary care team

Education

Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

Knowledge & Skills

Expertise in mobility training and rehabilitation techniques
Strong patient evaluation and treatment planning abilities
Effective communication and clinical documentation skills
Ability to adapt plans based on resident progress and interdisciplinary input
