Location: Merced, CA

Candidate Pay Rate: $85 - $59.5 per hour

Roles & Responsibilities

Evaluate residents and create individualized physical therapy treatment plans that promote mobility and independence
Deliver therapies focused on strength, balance, gait and functional recovery with compassion and clinical precision
Monitor patient progress, adjust interventions as needed, and document outcomes clearly and timely
Collaborate closely with physicians, therapists, nursing staff and the interdisciplinary team to optimize resident care
Educate residents and caregivers on safe mobility techniques, exercises and fall prevention strategies
Maintain accurate clinical documentation and contribute to quality improvement efforts within the SNF

Qualifications

Active California Physical Therapist license (required)
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program (required)
Previous SNF, rehabilitation, or geriatric therapy experience preferred
Strong clinical assessment and patient care skills with sound clinical reasoning
Excellent communication and teamwork skills to thrive in a multidisciplinary environment

Education

Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

Knowledge & Skills

Expertise in mobility training and rehabilitation techniques
Strong patient evaluation and treatment planning abilities
Skilled in clinical documentation and outcome tracking
Effective verbal and written communication with residents, families and care teams
Familiarity with SNF care processes and interdisciplinary collaboration
