Location: Madera, CA

Candidate Pay Rate: $85 - $59.5 per hour

Roles & Responsibilities

Evaluate residents and design individualized physical therapy treatment plans to improve mobility, strength, balance, and function
Deliver hands-on therapy and mobility training that helps residents regain independence and quality of life
Monitor resident progress and adjust treatment plans based on clinical findings and goals
Collaborate closely with physicians, nurses, occupational therapists, and interdisciplinary team members to coordinate care
Maintain accurate, timely clinical documentation and ensure regulatory compliance
Educate residents and caregivers on home and facility-based exercise programs, fall prevention, and safe mobility techniques

Qualifications

Active California Physical Therapist license (required)
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
Previous SNF, geriatric, or rehabilitation experience preferred
Strong clinical assessment and patient care skills with excellent documentation habits
Ability to work effectively within a multidisciplinary team and communicate clearly with residents and families

Education

Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

Knowledge & Skills

Expertise in mobility training and rehabilitation techniques
Proficient patient evaluation and treatment planning abilities
Strong communication skills for interacting with residents, families, and care teams
Skilled in clinical documentation and regulatory best practices
Familiarity with geriatric care principles and SNF workflows
