Roles & Responsibilities

• Evaluate residents and design individualized treatment plans that restore mobility and independence
• Provide skilled physical therapy focused on mobility, balance, gait training, strength, and fall prevention
• Treat residents recovering from orthopedic surgeries, strokes, fractures, and other medical conditions
• Collaborate with occupational therapy, speech therapy, nursing, and physicians to deliver coordinated care
• Educate residents and caregivers on therapeutic exercises and safe mobility techniques to support long-term progress
• Complete accurate and timely documentation in compliance with facility and state regulations

Qualifications

• Active California Physical Therapist License required
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Current CPR/BLS certification
• Prior SNF or geriatric rehabilitation experience preferred
• Strong communication and teamwork skills to work effectively with residents and interdisciplinary staff

Education

Knowledge & Skills

• Skilled in patient evaluation and developing targeted treatment plans
• Expertise in geriatric rehabilitation, mobility, balance, and gait training
• Strong documentation practices and regulatory compliance knowledge
• Ability to work collaboratively with OT, SLP, nursing, and physicians
• Effective communication with residents, families, and care teams
• Proficiency in fall prevention strategies and safe transfer techniques
