Roles & Responsibilities

• Assess patients' mobility, strength, balance, and functional needs to create individualized treatment plans
• Deliver hands-on therapy and progressive exercise programs to help patients regain independence
• Collaborate with nurses, therapists, and physicians to coordinate comprehensive care
• Document patient progress clearly and accurately in the medical record
• Educate patients and caregivers on home programs, safety, and strategies to maximize recovery
• Participate in discharge planning and community reintegration to support long-term outcomes

Qualifications

• Current Physical Therapist (PT) license in West Virginia or ability to obtain state licensure
• Basic Life Support (BLS) certification required; additional certifications a plus
• Clinical experience in inpatient rehab, acute care, or outpatient settings preferred (1+ year recommended)
• Strong clinical reasoning, patient-centered care approach, and accurate documentation skills
• Excellent communication and teamwork skills to thrive in a collaborative environment

Education

• Doctor of Physical Therapy (DPT) preferred; Master's or Bachelor's in Physical Therapy considered

Knowledge & Skills

• Patient assessment and treatment planning
• Therapeutic exercise, manual therapy, and gait training
• Neurological and orthopedic rehabilitation principles
• Assistive device training and fall prevention strategies
• Proficient documentation in electronic medical records (EMR)
• Patient and family education with clear, compassionate communication
