Roles & Responsibilities

• Implement treatment plans developed by the supervising Physical Therapist to help patients regain mobility
• Provide skilled therapy interventions including therapeutic exercise, gait training, balance activities, and transfer training
• Assist patients recovering from surgeries, strokes, fractures, and other medical conditions with safe, effective care
• Monitor patient progress and promptly report clinical changes to the supervising PT
• Educate patients and caregivers on home exercise programs and safe mobility techniques to support long-term outcomes
• Complete accurate and timely documentation in accordance with facility and state guidelines
• Collaborate with interdisciplinary team members to ensure coordinated, high-quality care

Qualifications

• Active California PTA license required
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR/BLS certification required
• Previous SNF or geriatric experience preferred
• Strong communication and teamwork skills; comfortable collaborating with nurses, therapists, and caregivers
• Ability to work compassionately with the geriatric population and support patient independence

Education

• Graduate of an accredited Physical Therapist Assistant program

• Gait training and safe ambulation strategies
• Balance activities and fall prevention approaches
• Transfer training and safe patient handling
• Accurate clinical documentation and outcome tracking
