Roles & Responsibilities

• Perform comprehensive patient assessments and evaluations to identify rehabilitation needs
• Develop and implement individualized plans of care with measurable short-term goals
• Provide skilled gait training, mobility training and safe transfers using appropriate assistive devices
• Monitor vital signs and strength testing to guide safe progression of therapy
• Document evaluations, plans of care, reassessments and discharge summaries in the charting system (Epic preferred)
• Lead discharge planning and coordinate with interdisciplinary team members for seamless transitions
• Educate patients and families on home exercise programs, safety and injury prevention to support long-term success
• Use time management and prioritization to manage a busy acute care caseload effectively

Qualifications

• Current California Physical Therapy license (required) — active license only; pending licenses not accepted
• BLS certification preferred
• Minimum 2 years of physical therapy experience (acute care experience highly relevant)
• Travel experience is required for this assignment
• Comfort managing caseloads and ratios consistent with acute care (patient ratios reported around 1:8)
• Proficiency with electronic charting (Epic preferred) and strong documentation skills
• Experience with mobility equipment and safe patient handling (Hoyer lift, sit-to-stand, gait belts, walkers)

Knowledge & Skills

• Patient assessment, evaluation and reassessments
• Plan of care development and discharge planning
• Gait training, mobility training and assistive device prescription
• Home exercise program design and patient/family education
• Strength testing, vital sign monitoring and cardiopulmonary considerations
• Experience across populations: geriatrics, neurology, oncology and orthopedics
• Skilled in acute care workflows and equipment (Hoyer lift, hospital/specialty beds, traction tables)
• Documentation best practices and familiarity with Epic charting
