Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
Administer medications and treatments accurately and document accordingly
Monitor and report changes in patient condition to the care team
Perform routine assessments (vitals, skin integrity, mobility) and ensure timely interventions
Collaborate with RNs, therapists, and support staff to deliver coordinated care
Maintain clear, professional documentation in the patient record
Support patient comfort, dignity, and education during daily care activities

Qualifications

Valid LPN license (required); ability to hold/transfer Pennsylvania licensure is preferred
Current BLS/CPR certification (or willingness to obtain)
Comfort working in a skilled nursing/long-term care environment
Clear background check and up-to-date immunizations as required by facility
Strong communication skills and a teamwork mindset

Education

LPN diploma or certificate; active LPN licensure

Knowledge & Skills

Medication administration and basic clinical assessments
Wound care basics and infection control practices
Electronic medical record (EMR) charting familiarity
Ability to prioritize tasks and manage time during an 8-hour shift
Compassionate patient interaction and family communication
