Roles & Responsibilities

• Provide compassionate direct patient care and assistance with activities of daily living
• Monitor residents and report changes in condition to the clinical team promptly
• Assist with safe transfers, mobility and positioning to promote resident comfort
• Maintain accurate, timely documentation of care and observations
• Support infection prevention practices and maintain a clean, safe environment
• Collaborate with nurses and interdisciplinary team members to deliver person-centered care

Qualifications

• Active Licensed Nursing Assistant (LNA) credential; T-LNA accepted
• Eligible or licensed to work in New Hampshire (and Vermont if covering VT shifts)
• Comfortable working the listed schedule: Monday, Tuesday, Thursday, Friday, Saturday
• Strong interpersonal skills and a team-first attitude
• Current basic life support (BLS) certification if required by facility

Education

• High school diploma or equivalent
• Completion of an approved LNA training program

Knowledge & Skills

• Skilled in assisting with ADLs, transfers and patient hygiene
• Working knowledge of charting and documentation best practices
• Familiarity with infection control procedures and safe patient handling
• Good observational skills and ability to communicate clinical changes clearly
• Ability to work efficiently in a fast-paced long-term care environment
