Location: Lebanon, NH

Contract Type: Per-Diem

Shift Type: TBD, 3:00 PM - 11:00 PM (Evening / day shift)

Hours per Week: 40

Candidate Pay Rate: $22 per hour

Roles & Responsibilities

Provide compassionate personal care and assistance with activities of daily living (ADLs)
Assist patients with mobility, transfers, bathing, dressing, and toileting needs
Take and accurately record vital signs and report changes to the nursing team
Support nursing staff with treatments, patient comfort, and timely documentation
Communicate clearly and courteously with patients, families, and colleagues
Maintain a safe, clean environment and follow infection prevention practices

Qualifications

Active Licensed Nursing Assistant (LNA) certification — VT or NH (as applicable)
Demonstrated compassionate patient care and teamwork
Ability to pass facility onboarding requirements (background check, health screening)
Reliable attendance and professional communication skills
Prior long-term care or acute care experience is a plus

Education

High school diploma or equivalent preferred
Completion of an approved nursing assistant training program and LNA certification

Knowledge & Skills

Basic patient care and safety practices, including infection control
Comfortable taking and recording vital signs and observing patient condition
Familiarity with care planning and documentation best practices
Strong interpersonal skills and the ability to work well in a team
Time management and the ability to prioritize patient needs
