Roles & Responsibilities

Conduct comprehensive psychosocial assessments to identify patient needs and strengths
Develop individualized treatment plans that are practical, evidence-based, and person-centered
Deliver evidence-based therapeutic interventions across the lifespan for conditions like depression, anxiety, trauma, and substance use
Collaborate closely with primary care providers, nurses, and behavioral health colleagues to coordinate whole-person care
Maintain timely, accurate clinical documentation to support continuity of care and compliance
Advocate for patients and connect them to community resources and supports when appropriate

Qualifications

Active LISW license in Iowa (required)
Master's degree in Social Work from an accredited program (required)
Minimum of 2 years of clinical social work experience preferred
Experience working in rural or medically underserved settings is a plus
Strong interpersonal, organizational, and clinical documentation skills

Education

Master's degree in Social Work (MSW) from an accredited program

Knowledge & Skills

Behavioral health interventions for depression, anxiety, trauma, and substance use
Integrated care collaboration with primary care teams
Clinical documentation best practices and regulatory awareness
