Roles & Responsibilities

Provide compassionate bedside care and monitor patient condition
Administer medications and treatments safely and accurately
Perform thorough assessments and document findings in the chart
Collaborate with the interdisciplinary care team to coordinate patient care
Educate patients and families on care plans and discharge instructions
Assist with procedures and ensure a safe, clean environment for patients
Escalate clinical changes and follow up on provider orders promptly

Qualifications

Current LPN license (state-appropriate licensure required)
Basic Life Support (BLS/CPR) certification — current
Clinical experience in a hospital or long-term care setting preferred
Ability to work 12.5-hour shifts and participate in rotation coverage
Up-to-date immunizations and willingness to complete background checks

Education

Completion of an accredited LPN/Vocational Nursing program (diploma or certificate)

Knowledge & Skills

Strong clinical assessment and vital signs monitoring skills
Proficient medication administration and charting practices
Comfortable using electronic medical records and documentation systems
Effective communication and teamwork with nursing staff and providers
Time management, prioritization, and safe patient handling techniques
