Roles & Responsibilities

• Conduct comprehensive psycho-social assessments to identify patient and family needs
• Develop and document individualized treatment plans that guide care and recovery
• Provide crisis counseling and timely interventions for patients in acute need
• Coordinate discharge planning, referrals, and linkages to community resources
• Collaborate actively in interdisciplinary team meetings to support patient-centered care
• Maintain timely, accurate documentation in the electronic health record (EHR)
• Advocate for patients with state and federal agencies to secure necessary services
• Support patients and families through education, coaching, and resource navigation

Qualifications

• Licensed Clinical Social Worker (LCSW) required at time of submittal (LMSW considered where appropriate)
• Minimum 2-4 years of social work experience in inpatient or outpatient healthcare settings
• Must be at least 21 years of age
• NABCS clearance and ICPA clearance required
• Flu vaccine required prior to assignment start
• Strong documentation skills and EHR proficiency
• Experience with crisis intervention, case management, and treatment planning

Education

• Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)

Knowledge & Skills

• Expertise in mental health evaluations and crisis counseling
• Solid understanding of case management principles in integrated care
• Proficiency with electronic health records, Google Drive, phone systems, and standard clinic tech
• Effective oral and written communication and empathic listening
• Ability to work collaboratively with multi-disciplinary teams and community agencies
• Familiarity working with individuals experiencing homelessness, addictions, and severe/persistent mental illness
• Knowledge of community support resources and discharge planning processes
