Roles & Responsibilities

• Provide compassionate direct patient care consistent with Licensed Practical Nurse scope
• Administer medications safely and accurately, following facility protocols
• Monitor and document patient condition, including vital signs and changes in status
• Collaborate with RNs, physicians, and interdisciplinary team members to deliver coordinated care
• Maintain clear, timely charting and handoffs in the facility’s documentation system
• Support patient and family education to promote understanding and recovery
• Help maintain a safe, clean, and therapeutic environment for residents

Qualifications

• Active LPN license (appropriate for practice in New Mexico) required
• Completion of an accredited practical nursing program
• Clinical experience in a long-term care or skilled nursing setting preferred
• Comfortable working 12+ hour day shifts and delivering hands-on patient care
• Reliable professional references and ability to complete required facility onboarding

Education

• Graduation from an accredited LPN (practical nursing) program; high school diploma or equivalent

Knowledge & Skills

• Strong medication administration and documentation skills
• Proficiency in basic patient assessments and monitoring
• Familiarity with infection control and long-term care best practices
• Clear communication and teamwork skills for working with interdisciplinary teams
• Good time management and the ability to prioritize patient needs
