Roles & Responsibilities

• Provide compassionate, resident-centered nursing care that preserves dignity and comfort
• Administer medications and treatments accurately and in accordance with facility policy
• Perform thorough assessments and document changes in condition clearly and promptly
• Collaborate with the interdisciplinary care team to create and follow personalized care plans
• Educate residents and families about care plans, medications, and health promotion
• Maintain a safe, clean environment and adhere to infection control standards
• Support smooth shift handoffs and mentor colleagues when appropriate

Qualifications

• Active Licensed Practical Nurse (LPN) license — state-appropriate and in good standing
• Previous LPN experience in long-term care, skilled nursing, or similar settings preferred
• Basic Life Support (BLS/CPR) certification preferred or required upon hire
• Strong clinical judgment, attention to detail, and a heart for teamwork
• Ability to pass required background checks and health screenings

Education

• Completion of an accredited practical nursing program or equivalent credential

Knowledge & Skills

• Medication administration, documentation, and safety practices
• Skilled in resident assessments, vital signs, and basic wound care
• Comfortable with electronic medical record charting and clinical documentation
• Excellent communication skills for collaborating with residents, families, and staff
• Time management and prioritization in a fast-paced care environment
