Shift Type: Night shift, 6:45 PM - 7:15 AM (36-hour guarantee); specific days assigned by facility (schedule determined at time of submission)
Hours per Week: 38
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Assess patients' conditions regularly and deliver skilled nursing care following the physician-authorized plan of care
- Initiate and revise individualized plans of care, adapting interventions as patient needs change
- Provide medication management and monitor therapeutic responses to ensure safe, effective treatment
- Coordinate care with CNAs and the interdisciplinary team to optimize outcomes and continuity
- Document clinical findings and interventions accurately in the medical record (Epic)
- Communicate changes in patient condition to physicians and obtain new orders as needed
- Provide on-going feedback and education to team members to support patient-centered care
Qualifications
- Active Michigan RN license (LARA and/or Nursys verification required) — must be active
- Minimum 2+ years of Long Term Care (LTC) experience (required)
- Current American Heart Association BLS (AHA) certification
- Completed skills checklist (current within 1 year) relevant to LTC
- Two supervisory reference forms completed within the last two years
- Comfortable using Epic for documentation and charting
Knowledge & Skills
- Patient assessment and evaluation with strong clinical judgment
- Care planning and individualized interventions
- Medication management and monitoring for therapeutic response
- Care coordination across interdisciplinary teams
- Accurate, timely documentation in Epic
- Familiarity with LTC workflows and regulatory standards
- Effective communication with physicians, CNAs, patients, and families
