Roles & Responsibilities

• Conduct comprehensive assessments and design individualized service plans to support high-risk patients
• Coordinate care and resource acquisition across inpatient, outpatient, and community providers
• Lead discharge planning to ensure safe transitions and reduce care fragmentation
• Perform utilization review and assess medical necessity for specialty referrals
• Monitor client progress, respond to emerging needs, and adjust plans to achieve measurable outcomes
• Collaborate with physicians, nurses, social workers, PRC department, and other providers to implement effective care plans
• Document thoroughly in the electronic health record to support continuity and quality of care

Qualifications

• MSW option: Master of Social Work (MSW) with LMSW or LCSW licensure (valid, current, full and unrestricted in any US state, territory, or Puerto Rico)
• RN option: Associate degree in Nursing (must obtain BSN within 2 years of hire) with a current, full and unrestricted RN license in any US state, territory, or Puerto Rico
• Minimum 3 years of experience in a clinical care setting
• Experience in discharge planning, case management, or utilization review processes
• Demonstrated knowledge of electronic health record systems
• Current Basic Life Support (BLS) certification by the American Heart Association
• Fingerprinting must be submitted and cleared prior to start

Education

• MSW (Master of Social Work) for Social Worker role
• Associate Degree in Nursing for RN Case Manager role; BSN required within 2 years of hire

Knowledge & Skills

• Assessment, service planning, and resource acquisition
• Care coordination across inpatient and outpatient settings
• Utilization review and medical necessity evaluation
• Discharge planning best practices
• Familiarity with Electronic Health Records (EHR) and documentation standards
• Ability to work effectively with diverse cultural populations, including Navajo, Hopi, and Southern Paiute communities
• Strong communication and multidisciplinary collaboration skills
