Roles & Responsibilities

• Provide compassionate bedside care to assigned patients, ensuring dignity and comfort
• Administer medications and treatments per care plan and scope of practice
• Monitor and document vital signs, patient status changes, and response to interventions
• Collaborate with RNs, physicians, and the care team to implement individualized care plans
• Perform wound care, basic procedures, and ADL assistance as needed
• Maintain accurate, timely documentation in the electronic health record
• Educate patients and families on care, medications, and discharge instructions
• Advocate for patient needs and contribute to a positive patient experience

Qualifications

• Active Licensed Practical Nurse (LPN) licensure (required)
• Current Basic Life Support (BLS) certification, or willingness to obtain
• Recent clinical experience as an LPN preferred
• Strong communication and teamwork skills
• Ability to complete documentation and follow facility policies

Education

• LPN diploma or certificate from an accredited program
• High school diploma or equivalent

Knowledge & Skills

• Solid knowledge of medication administration and basic pharmacology
• Proficient in patient assessment, vital signs, and recognizing changes in condition
• Comfortable with wound care and common nursing procedures within LPN scope
• Clear, professional documentation skills in an electronic health record
• Ability to prioritize tasks in a fast-paced clinical environment
• Compassionate patient-centered approach and strong interpersonal skills
