Roles & Responsibilities

• Provide compassionate direct care to residents, assisting with activities of daily living
• Take and document vital signs and observe changes in condition
• Assist with mobility, transfers and safe patient handling
• Follow individualized care plans and report concerns to the nursing team
• Maintain clean, safe and respectful environment for residents
• Support admissions, discharges and smooth transitions of care
• Communicate clearly with team members and families to promote resident wellbeing

Qualifications

• Current Licensed Nursing Assistant (LNA) credential (NH or VT licensure preferred)
• CPR/BLS certification or willingness to obtain upon hire
• Comfortable working per diem shifts and adapting to facility needs
• Ability to pass background check and required health screenings/vaccinations
• Strong communication skills and a caring, professional demeanor
• Previous long-term care or geriatric experience is a plus

Education

• High school diploma or GED and completion of a state-approved LNA training program

Knowledge & Skills

• Skilled in patient hygiene, feeding assistance and toileting support
• Ability to take and accurately record vital signs and simple observations
• Familiarity with infection prevention and safe patient handling
• Comfortable with electronic or paper charting and documentation
• Team-oriented, organized and able to prioritize tasks in a busy setting
• Empathy, patience and strong interpersonal skills with residents and families
