Roles & Responsibilities

Provide compassionate, hands-on patient care as a Licensed Practical Nurse
Administer medications and treatments safely and accurately per orders
Perform routine assessments and report changes in patient condition promptly
Document care and maintain accurate charts to support excellent continuity
Collaborate with RNs, physicians and interdisciplinary team members to deliver great outcomes
Support patients and families with clear communication and emotional support

Qualifications

Valid LPN license (required)
Current, relevant clinical experience as an LPN (per diem experience welcome)
Reliable, professional, and able to work a 12+ hour day shift when scheduled
Comfortable working as part of a collaborative care team

Education

Completion of an accredited LPN/Vocational Nursing program (diploma or certificate)

Knowledge & Skills

Strong fundamentals in patient care and medication administration
Skilled at bedside assessments and basic nursing procedures
Clear, compassionate communication with patients and team members
Good clinical documentation and basic EHR familiarity
Ability to prioritize tasks in a busy care environment
