Roles & Responsibilities

Provide compassionate, patient-centered nursing care to assigned residents
Administer medications and treatments according to facility policies and physician orders
Assess and monitor patient status, documenting changes and interventions accurately
Collaborate with nurses, physicians, and interdisciplinary team members to coordinate care
Educate patients and families on care plans, medications, and follow-up needs
Maintain a safe environment through infection control practices and safety checks
Contribute to thorough, timely charting and handoffs to ensure continuity of care

Qualifications

Current Licensed Practical Nurse (LPN) license for Delaware (required)
BLS/CPR certification (required or obtained upon hire depending on facility policy)
Previous clinical nursing experience preferred; confidence in med administration and basic assessments
Strong communication skills and ability to work effectively within a team
Ability to handle physical demands of nursing and adapt to a busy care environment

Education

Graduate of an accredited practical/vocational nursing program
Current LPN license for Delaware

Knowledge & Skills

Proficient in patient assessment and monitoring
Skilled in medication administration and documentation
Comfortable using electronic health records and charting systems
Knowledge of infection control and safety protocols
Good time management, prioritization, and teamwork abilities
