Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents, focusing on dignity and comfort
• Administer medications and treatments accurately and on time
• Perform resident assessments, monitor vital signs and report changes promptly
• Document care thoroughly in the electronic health record to ensure continuity
• Collaborate with nurses, CNAs and interdisciplinary team members to develop and follow individualized care plans
• Assist with admission, transfer and discharge processes while educating residents and families
• Support a safe, clean environment by following infection control and safety protocols

Qualifications

• Current, active LPN license in Pennsylvania (or compact/licensure as applicable)
• Completion of an accredited practical nursing program (diploma or certificate)
• Basic Life Support (BLS/CPR) certification required; additional certifications welcomed
• Background check, drug screen and immunizations per facility policy
• Previous experience in long-term care or skilled nursing preferred but new grads are welcome to apply

Education

• Graduation from an accredited practical nursing program; LPN/LVN diploma or certificate required

Knowledge & Skills

• Strong medication administration and documentation skills
• Solid patient assessment and clinical judgment abilities
• Comfortable using EHR/charting systems and following care protocols
• Excellent communication and teamwork skills with residents and families
• Time management and prioritization in a fast-paced clinical setting
• Knowledge of infection prevention and standard nursing procedures
