Roles & Responsibilities

• Assess and evaluate speech, language, cognitive-communication, and swallowing disorders to create individualized care plans
• Develop and implement evidence-based therapy plans that target functional communication goals
• Document progress, treatment plans, and discharge summaries in the facility’s EHR clearly and promptly
• Collaborate with interdisciplinary team members to coordinate care and optimize patient outcomes
• Educate patients and families on strategies to support communication and swallowing safety at home
• Provide timely clinical recommendations and participate in care planning meetings

Qualifications

• Licensed Speech Language Pathologist (SLP) — current state license or eligibility to obtain one
• Master’s degree in Speech-Language Pathology (or equivalent)
• ASHA CCC-SLP preferred or active pursuit of certification
• Clinical experience in adult/rehab settings (experience levels considered — discuss with recruiter)
• Strong documentation skills and comfort using electronic health record systems

Education

• Master’s degree in Speech-Language Pathology (required)
• ASHA CCC-SLP (preferred)

Knowledge & Skills

• Skilled in standardized and informal assessment techniques for communication and swallowing
• Expertise in treatment planning, goal writing, and progress measurement
• Knowledge of dysphagia management and safe swallowing strategies
• Excellent interpersonal and communication skills for working with patients, families, and care teams
• Familiarity with regulatory and documentation standards in clinical settings
