Roles & Responsibilities

• Evaluate patients’ functional abilities and develop individualized occupational therapy plans
• Deliver skilled OT interventions to improve independence in daily living and mobility
• Document assessments, treatment plans and progress notes in a timely, professional manner
• Collaborate closely with nurses, PT/SLP and the interdisciplinary team to coordinate care
• Educate patients and caregivers to support safe transitions and long-term progress
• Maintain productivity expectations and follow CRG policies and procedures
• Be present on-site as required to receive guaranteed hours and represent the therapy team professionally (black scrubs)

Qualifications

• 1-3 years of occupational therapy experience (as indicated in the job posting)
• Active Occupational Therapist licensure or eligibility (state licensure required upon hire)
• Degree in Occupational Therapy (OTR or equivalent) — completed from an accredited program
• Strong documentation skills and ability to meet productivity expectations
• Comfort working in a skilled nursing facility (SNF) setting and collaborating with a multidisciplinary team
• Ability to comply with facility policies, dress code (black scrubs), and on-site attendance requirements

Knowledge & Skills

• Skilled in functional assessments and developing meaningful, goal-focused treatment plans
• Experience in SNF clinical practices and resident-centered care
• Excellent communication and patient/caregiver education skills
• Proficient clinical documentation and time-management to meet productivity targets
• Team-oriented approach and ability to collaborate across disciplines
